John Smith, having filed an application for admission to the bar of Colorado, do hereby consent to have an investigation made by or on behalf of the Colorado Office of Attorney Regulation (OARC), Office of Attorney Admissions, and/or the Colorado Supreme Court Character and Fitness Committee as to my moral character, reputation and fitness for the practice of law. I agree to give any further information which may be required in reference to my past record.
I authorize and request every person, firm, company, corporation, association, attorney, creditor, bar association, employer (past or present), landlord (past or present), credit reporting agency, governmental agency, law enforcement agency, school, college, university, educational institution, and any other agency having control of documents, records, files, writings, or any other information pertaining to me to provide to the Office of Attorney Regulation Counsel/Office of Attorney Admissions any such information (whether dismissed, erased, sealed, expunged, confidential, formal, informal, pending, or closed), including, but not limited to, information regarding any and all debts, charges, defaults, payment history, tax liabilities, complaints, disciplinary actions, grievances, sanctions, suspensions, reprimands, disqualifications, censures, resignations, terminations, citations, arrests, indictments, convictions, judgments, court-martial, non-judicial punishments, administrative discharges, juvenile offenses, employment, legal actions, service, licensure, or any other data or information pertaining to my character and fitness under C.R.C.P. 208. I permit the OARC, its members, employees, agents, and/or representatives to inspect and make copies of any such information and on its own volition or in response to any inquiry from any agency of the Colorado Supreme Court or to an authorized agency under C.R.C.P. 211.1 with the information, documents, or records contained in my file.
I authorize the OARC/Office of Attorney Admissions to obtain any information from my official record on file with the Selective Service System, and further authorize the Department of the Army, Navy or Air Force to furnish the OARC/Office of Attorney Admissions the record of each period of my service therein and to furnish my service record for each period; and do hereby consent to and authorize the release of such information by the Selective Service System and the Department of the Army, Navy or Air Force.
Pursuant to C.R.C.P. 222.5(1), I agree that persons performing official duties under the provisions of the Rules Governing Admission to the Practice of Law in Colorado, including but not limited to the Advisory Committee and its members, the Board of Law Examiners and its members, the Attorney Regulation Counsel and staff, the Presiding Disciplinary Judge and staff, members of hearing boards, and other enlisted volunteers are immune from suit for all conduct performed in the course of their official duties.
I further agree that under C.R.C.P. 202.5(2) that testimony, records, statements of opinion and other information regarding me and my application for attorney admission communicated by any person or entity to the Advisory Committee or its members, the Board of Law Examiners or its members, the Attorney Regulation Counsel or staff, the Presiding Disciplinary Judge or staff, members of hearing boards, the Colorado Lawyer Assistance Program or staff, or other volunteers shall be absolutely privileged, and no lawsuit shall be predicated thereon.
